My Choice offers career and community exploration, assessments, career planning, adult day services, and individual employment supports for adults with disabilities. They serve individuals throughout the Cleveland area. They also offer social and fitness outings. They accept I/O and Level 1 Waivers, and they can also accept private pay, if it is needed.
